A) Employees setting trusses on the second floor walls of a multi-family home were not using any form of fall protection when exposed to falls greater than 6'.    B) Employees working on the floor of the second floor were not protected from falls to the ground by completed or guarded gable end walls.      Mark Gajeski Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, or an equivalent Standard, which was contained in OSHA Inspection Number 312552359, Citation #1, Item #1, which was affirmed as a Final Order on 6/7/10, with respect to a workplace located at 2017 Morning Dew Ln, De Pere, WI 54115.     Mark Gajeski Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, or an equivalent Standard, which was contained in OSHA Inspection Number 309839637, Citation #1, Item #1A, which was affirmed as a Final Order on 9/20/07, with respect to a workplace located at 409 Wooded Shoe Cir., Kaukauna, WI 54130.
