29 CFR 1910.213(h)(4):Installation was not in such a manner that the front end of the unit will be slightly higher than the rear, so as to cause the cutting head to return gently to the starting position when released by the operator: (a) In the carpenters' shop on or about May 10, 2012, and at times prior thereto, the cutting head did not return to the starting position on a Craftsman 10 inch Radial Arm Saw, model number 315220381, serial number 051257778. It appeared that the return cable was remove from the saw's cutting head.

29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: (a) In the auto shop on or about May 10, 2012, and at times prior thereto, the work rest was one half inch from the right side abrasive wheel on a Dayton 6 Variable Speed Bench Grinder. The wheel was used to grind bolts, tools and metal fabricated parts.

29 CFR 1910.215(b)(9): The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch: (a) In the auto shop on or about May 10, 2012, and at times prior thereto, the tongue guard was 1-1/4 inches from the right side abrasive wheel on a Dayton 6 Variable Speed Bench Grinder. The wheel was used to grind bolts, tools and metal fabricated parts.
