29 CFR 1910.106(d)(3)(ii)(a): Metal cabinets constructed in the following manner shall be deemed to be in compliance. The bottom, top, door, and sides of cabinet shall be at least No. 18 gage sheet iron and double walled with 1 1/2 - inch air space. Joints shall be riveted, welded or made tight by some equally effective means. The door shall be provided with a three-point lock, and the door sill shall be raised at least 2 inches above the bottom of the cabinet. Two metal storage cabinets in the morgue area containing flammable liquids including ethanol, ethyl acetate, and acetone did not have a functioning three-point lock.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): Machine specific procedures developed for equipment including but not limited to exhaust fan ex26, air handler HVAC25, and air handler 14 did not specifically and clearly outline the techniques to be utilized for the control of hazardous energy. ABATEMENT NOTE: 1910.147(c)(4)(ii)(A) A specific statement of the intended use of the procedure; 1910.147(c)(4)(ii)(B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 1910.147(c)(4)(ii)(C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 1910.147(c)(4)(ii)(D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.
