29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. On May 10, 2012, employees were exposed to fall hazards of up to approximately 30 feet while performing roofing activities without appropriate fall protection in place. The Riverbend Roofing, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection number 312885775, citation number 1, Item 1 and was affirmed as a final order on April 27, 2009, with respect to a workplace located at 4341 18th Avenue, Rock Island, IL 61201.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Employees were exposed to fall hazards while performing roofing activities near an unguarded hole in the roof because employees were not protected from falling to lower levels by personal fall arrest systems, covers, or a guardrail system.
