29 CFR 1910.215(a)(4):     Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    (a)  At the worksite near the welders shop, on May 10, 2012, the left work rest for the Dayton Electric bench grinder (model 2LKR6) was adjusted approximately 1/2 inch away from the grinding wheel.  This condition exposed employees to struck-by hazards.    (b)  At the worksite near the welders shop, on May 10, 2012, the right work rest for the Dayton Electric bench grinder (model 2LKR6) was adjusted approximately 1/2 inch away from the grinding wheel.  This condition exposed employees to struck-by hazards.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    At the worksite near welders shop, on May 10, 2012, the left tongue guard for the Dayton Electric (model 2LKR6) was adjusted approximately 1 away from the grinding wheel.  This condition exposed employees to struck-by hazards.    At the worksite near welders shop, on May 10, 2012, the right tongue guard for the Dayton Electric (model 2LKR6) was adjusted approximately 1 away from the grinding wheel.  This condition exposed employees to struck-by hazards.

29 CFR 1910.219(d)(1):     Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    (a)  At the worksite in the middle of the facility, on May 10, 2012, the pulley for the large Lockforming 'roll forming' machine was not equipped with a guard.  This condition exposed employees to caught-by hazards.      (b)  At the worksite in the middle of the facility, on May 10, 2012, the pulley for the small Lockforming 'roll forming' machine was not equipped with a guard.  This condition exposed employees to caught-by hazards.

29 CFR 1910.305(g)(1)(iv)(A):     Flexible cords and cables were used as a substitute for the fixed wiring of a structure:    At the worksite, on May 10, 2012, the 4 hanging fluorescent lighting fixtures were wired with flexible cords.    At the worksite, on May 10, 2012, 2 flexible cords were hanging from the ceiling and used as a power source for employees.
