Fuel station: On or about March 7, 2012 and at times prior to; the employer exposed his employees to recognized hazards in that employees were allowed to operate forklifts without fastening manufactured seat belts.

Fuel station: On or about March 7, 2012 and at times prior to; the employer exposed his employees to a potential fire hazard in that employees were allowed to operate crane number 954 which was leaking hydraulic fluid during normal operations.

Fuel station: On or about March 7, 2012 and at times prior to; the employer exposed his employees to electrical hazards in that employees were allowed to work around an electrical receptacle which was not properly connected to prevent strain relief for the joints and terminal screws.
