This violation was observed on or about May 9, 2012, on the east side of a newly built house where employees were exposed to an electrical hazard when required to use a flexible cord without a splice or tap.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation:    The employer does not provide the scaffold with base plates, mud sills or other adequate firm foundation.  This violation was observed on or about May 9, 2012, on the east side of a newly built house where employees were exposed to a fall hazard when required to work on the platform at a height of 13 feet without base plates, mud sills or firm foundation.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, stucture, personnel hoist, or similar surface was not used.      The employer does not provide employees with access to the scaffold..  This violation was observed on or about May 9, 2012, on the east side of a newly built house where employees were exposed to a fall hazard when accessing the platform without a ladder.

29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms.    The employer does not install guard rails on the scaffold.  This violation was observed on or about May 9, 2011, on the east side of a newly built house where employees were exposed to a fall hazard when required to nail vinyl siding from a platform of 13 feet without a guard rail system.

29 CFR 1926.452(j)(2):     Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    The employer does not require that pump jack scaffold be secured to the structure by rigid trianagular bracing at the bottom, top and other points as necessary.  This violation was observed on or about May 9, 2012 in the residential housing area where employees were exposed to a fall hazard when required to work from the pump jack scaffold without being secured to the structure.
