On or about May 10, 2012 at the work site located at Horizontal Well Drilling Rig #2, HWY 281 Jacksboro, Texas: the employer did not ensure that all trailers including but not limited to the Shale Shaker Trailer were grounded.

On or about May 10, 2012 at the work site located at Horizontal Well Drilling Rig #2, HWY 281 Jacksboro, Texas:  employees assigned to work on the monkey board were not provided an adequate means of safe egress from the work platform exposing employees to fractures, burns, and death.      HORIZONTAL WELL DRILLERS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 5(a)(1), WHICH IS CONTAINED IN OSHA INSPECTION 311872816, CITATION NUMBER 001, ITEM 2, ISSUED APRIL 2, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT PUNXY HC 3H, LAWRENCE TWP, CLEARFIELD CNTY, PENFIELD, PA.

29 CFR 1910.23(c)(1):  Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides.      On or about May 10, 2012, at the work site located at Horizontal Well Drilling Rig #2, HWY 281 Jacksboro, Texas:  a section of the top rail and mid rail were missing on the drill deck  leaving an opening of approximately 24 inches exposing employees to a fall hazard of more than 12 feet.        HORIZONTAL WELL DRILLERS WAS PREVIOULSY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.23(c), WHICH IS CONTAINED IN OSHA INSPECTION NUMBER 314157884, CITATION NUMBER 1, ITEM 2, ISSUED OCTOBER 20, 2010.

On or about May 10, 2012, at the work site located at Horizontal Well Drilling Rig #2, HWY 281 Jacksboro, Texas:  the employer did not ensure that the employees wore Personal Protection Equipment while engaged in oil and gas drilling activities.  The employees did not have on Flame-Resistant Clothing (FRC) exposing employees to flash fires.      HORIZONTAL WELL DRILLERS WAS PREVIOULSY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.132(a), WHICH IS CONTAINED IN OSHA INSPECTION NUMBER 313373540, CITATION NUMBER 1, ITEM 2, ISSUED AUGUST 11, 2010.
