OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from fall from heights hazards: At the worksite: On or about April 15, 2015, the employer allowed the use of an emergency escape device system (Geronimo) that was not installed per the manufactures requirements. An employee was exposed to the hazard of fall from heights.

29 CFR 1910.303(b)(3)(ii)(A): All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel were not ground fault circuit-interrupter protection for personnel: At the worksite: On or about April 15, 2015, and at times prior thereto, the employer allowed employees to use a flexible cord set (extension cord) connected to a receptacle that was not protected by a ground fault circuit-interrupter to power a sump pump outside in the mud pump area. Employees were exposed to the hazards associated with electrical shock.
