29 CFR 1910.95(d)(3):     Repeat employee exposure monitoring was not performed whenever a change in production, process, equipment or controls increases noise exposure:    a. Hose Line - Employees, who were placing hose to be shredded on a conveyor belt, were exposed to continuous noise at 125.4% of the exposure level of 90 dBA or an eight hour time weighted average (TWA) noise exposure of 91.7 dBA.  This exposure was observed occurring over a 476 minute sampling period o n 6/11/12.  A zero exposure was assumed for the 4 minutes not sampled.  The employer did not perform exposure monitoring when operation was moved to a new site, to determine if the hearing protection was adequate for the job performed.    b. Dry Line, Bail Buster - Employee, who was operating the bail buster on the dry line, was exposed to continuous noise at 62.7% of the exposure level of 90 dBA or an eight hour time weighted average (TWA) noise exposure of 86.5 dBA.  This exposure was observed occurring over a 472 minute sampling period o n 6/11/12.  A zero exposure was assumed for the 8 minutes not sampled.  The employer did not perform exposure monitoring when operation was moved to a new site, to determine if the hearing protection was adequate for the job performed.    c. Dry Line - Employees, who were picking on the dry line, were exposed to continuous noise at 57.9% of the exposure level of 90 dBA or an eight hour time weighted average (TWA) noise exposure of 85.9 dBA.  This exposure was observed occurring over a 445 minute sampling period o n 6/11/12.  A zero exposure was assumed for the 15 minutes not sampled.  The employer did not perform exposure monitoring when operation was moved to a new site, to determine if the hearing protection was adequate for the job performed.    d. Dry Line - Employee, who was operating a fork lift for the dry line, was exposed to continuous noise at 112.5% of the exposure level of 90 dBA or an eight hour time weighted average (TWA) noise exposure of 90.85 dBA.  This exposure was observed occurring over a 478 minute sampling period o n 6/11/12.  A zero exposure was assumed for the 2 minutes not sampled.  The employer did not perform exposure monitoring when operation was moved to a new site, to determine if the hearing protection was adequate for the job performed.
