Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification: On site at 7701 Reynolds Road, Mentor, Ohio: Two junction boxes, designed for permenant installation, were used as extension cords to power equipment incluidng but not limited to a Milwaukee Saws-All and a DeWalt radial saw.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired: On site at 7701 Reynolds Road, Mentor, Ohio: The outer jacket of insulation on an extension cord used by employees was damaged, exposing the inner conductors of the cord.

29 CFR 1926.503(a)(2)(ii): The employer did not ensure that employees were trained, by a qualified perosn, in the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used: On site at 7701 Reynolds Road, Mentor, Ohio: Two separately-installed anchors used by employees for personal fall arrest systems were not installed as per the manufacturer's instructions which stated that every hole on the anchors (32 of them) needed to be filled with 12x2 inch screws. One anchor had 4 screws and the other had 6 screws.
