29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Jobsite: An employee performing roofing work at the roof's edge and in front of the warning line, at a height of approximately 20 feet from the ground, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, for a jobsite located in Smithfield RI on or about 5-7-12.  All Phase Restoration, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 314406661, citation number 1, item number 2 and was affirmed as a final order on 12-12-11 with respect to a workplace located at 204 Bloomfield Ave in West Hartford CT.
