29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).    The following scaffold platforms, from which emplpoyees were engaged in construction activities, were not fully planked:    A. West Side Scaffold:    1. The single 18 3/4 inch wide platform used as a walking/working surface on the top of the second stage scaffold frame 13 feet 7 inches above the ground.    2. The two 18 3/4 inch wide platforms used as a walking/working surface on top of the first stage scaffold frame 7 feet 4 inches above the ground.    B. East Side Scaffold:    1. The single 18 3/4 inch wide platform used as a walking/working surface on the top of the second stage scaffold frame 13 feet 10 inches above the ground.    2. The single 18 3/4 inch wide platform used as a walking/working surface on the top of the stage scaffold frame 7 feet 7 inches above the ground.

29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames did not have a maximum spacing between rungs of 16 3/4 inches (43 cm). Non-uniform rung spacing caused by joining end frames together is allowed, provided the spacing does not exceed 16 3/4 inches (43 cm).    Jobsite: The spaces between the integral prefabricated scaffold rungs used to access the scaffold platforms on the east and west side scaffolds were 18 3/4 inches apart.

The scaffold erected on the west side of the establishment's entrance.    2. The scaffold erected on the east side of the establishment's entrance.

The 18 3/4 inch wide scaffold platform on the west side scaffold with a 13 foot 7 inch fall distance.     2. The 18 3/4 inch wide scaffold platform on the east side scaffold with 13 foot 10 inch fall distance.
