The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:    a. On or about May 9, 2012 at the above addressed jobsite employees operating the White 60 fork lift had not been certified to assure their qualifications for fork truck operations.

Current employees, and new employees prior to starting work, shall be trained in at least the following:    a. Such locks and tags shall be removed in accordance with established procedures only by the employee installing them or, if unavailable, by his or her supervisor:    a. On or about May 9, 2012 at the above addressed jobsite, a lock out/tag out program had not been developed to protect employees from inadvertent activation of equipment being worked on such as bucket elevators and augers.
