29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Manufacturing Area: The ingoing nip point, created by the 1 3/4 inch diameter, 42 inch long metal intake/feed rollers on the Post Machinery Company #056 Crusader Royal glue and box folding machine was not equipped with a guard. On or about May 03, 2012, the operator operating the above machine got one of hands and fingers into the turning rollers and sustained severe injuries from the incident.
