29 CFR 1910.147(e)(1):     Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact.    Facility located at 1131 Dakota Avenue, Dakota City, NE; specifically on the Slaughter Floor:  An employee failed to remove a lockout chain that was used to support the Split Saw Stand (remove gravity while servicing).  The machine was actuated causing the chain to break and metal components to spring free (a connecting chain link).      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
