Mulberry in Kansas City, MO, employees were exposed to serious fall hazards while working on scaffold platforms that consisted of single 18 inch wide fabricated scaffold planks.

Mulberry in Kansas City, MO, workers were exposed to serious fall and/or scaffold collapse hazards while working at various levels on a six section high tubular welded frame saffold where the bottom scaffold poles lacked base plates and were sitting directly on a plywood overlay.

Mulberry in Kansas City, MO, employees were exposed to serious fall hazards where the guardrail systems along the rear (south) sides consisted of solely of cross bracings which lacked either top or midrails depending on the platform level, and large openings existed along the west ends.
