29 CFR 1926.502(d)(15):     Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:    Roof Area: Two employees while working on the 6 in 12 sloped roof exposed to a fall of approximately 9 feet were both tied off to a single 3M Ridge Clamp which was marked as a single user anchor as determined on 5-11-12.    CERTIFICATION ABATEMNENT IS REQUIRED FOR THIS ITEM.

Roofing Project: Employees were exposed to falls from the front side of the roof of approximately 9 feet while walking / working from the 6 in 12 sloped roof as observed on 5-11-12.    ABATEMET DOCUMENTATION REQUIRED FOR THIS ITEM    SHEETS HANDYMAN SERVICE WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION 315223909, CITATION NUMBER 1,  ITEM NUMBER 4, ISSUED ON 5-17-2011.  THIS ITEM BECAME FINAL ORDER ON 6-1-2011 AND ABATEMENT WAS CERTIFIED ON 5-27-2011WITH RESPECT TO A WORKPLACE LOCATED AT 110 EAST REYNOLDS, BELLE, WEST VIRGINIA 25015.
