29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  (a)  929 Montrose Drive, South Charleston, WV:  On or about July 16, 2018 the employer's safety and health program did not address fall hazards in that employees were found working on an 8 on 12 sloped roof at heights greater than six feet without an approved fall protection system in place.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  929 Montrose Drive, South Charleston, WV:  On or about July 16, 2018 employees were installing roof felt paper on a residential house with an eight on twelve slope and fall heights between ten and twenty-five feet high and they were not protected with an approved fall protection system in that only slide guards were used.
