29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: Workplace: The employer did not ensure through visual observation, specific instruction and appropriate training that the two (2) employees who were performing maintenance on a pump had taken all necessary and appropriate steps and procedures to relieve all potentially hazardous stored energy prior to beginning work. On or about 04/03/12, hot liquid asphalt sprayed out onto the two (2) employees while they were performing the above task, and the two (2) employees sustained minor burns from the incident.
