29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required. The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a/ Gregor & Friends, LLC. Avenel, NJ site - Warehouse: Forklift operators who were trained by their previous employer or other trainers were not certified by the current employer to ensure the forklift training previously provided was adequate.
