When an employee was on the fire escape ladder to climb down to the ground floor, the ladder slid down and the employee fell to the ground. The fire escape ladder shifted and extended downwards while being occupied by an employee. On or about 05/02/12.    NOTE:  IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
