29 CFR 1910.305(b)(2)(i):     Pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose    West end of melon conveyor line, center area of the workplace: On 05/23/2012, employees engaged in workplace activities where exposed to shock hazards from contact with the following equipment:    a. Control Box Panelboard--one of two hinged covers for the device was fabricated from 1/4 inch plexiglass and was not approved for the purpose. The cover measured 34 inches long, 23 inches wide and was located at a height of 80 inches above the workplace floor. The panelboard contained electrical wiring that operated at 480 volts AC.    b. Junction box with two toggle switches--the energized toggle switches installed within a junction box did not contain a cover; operated at 120 volts AC, and were located at a height of 78 inches above the workplace floor. The two switch devices missing a cover contained exposed, energized connection side terminals mounted 1/2 inch behind the face of the devices.

29 CFR 1904.29(b)(2):     The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.    Throughout the workplace: On or about 05/23/2012, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out or incorrectly filled for the calendar year 2012.     a. OSHA Form 301: The form used to document the employee injury on 04/30/2012 did not contain the identity, title, telephone number or date of the person completing the form. The form also did not contain the Case Number from the Log, or the time that the employee began work on the day of the injury.    b. Equivalent OSHA Form 301("Employers First Report of Injury or Illness"): The form used to document the employee injury on 04/30/2012 did not contain the Name nor Address where the Injury or Exposure occurred (if incident occurred on a business site), the cause of the Injury, a List of Witnesses, and whether or not the employee was recruited in Texas. The equivalent form also did not contain the length of service in current position, the length of service in Occupation, the Signature of the Person completing the form, and the date that the form was signed.
