29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. a) Lot #427, Winchester Road, Upper Saint Clair, Pennsylvania, 15241: On or about May, 10, 2012, employees installing synthetic roofing underlayment on a two story residential dwelling were not protected by guardrail systems, safety net systems, or personal fall arrest systems. The employees were exposed to a fall hazard of approximately 18 feet to the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Lot #427, Winchester Road, Upper Saint Clair, Pennsylvania, 15241: On or about May 10, 2012, employees installing working on a steep roof (10 in 12 vertical to horizontal) were not trained in the recognition of fall hazards and the procedures to be followed in order to minimize these hazards.
