29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that: On or about 05/09/2012, at the jobsite at 1400 Breeze Ridge Dr., employee engaged in sewer pipe installation in an 11.6 feet deep excavation, was not provided with either sloping, benching, or shield system to protect employee against the hazard of a cave-in.
