29 CFR 1910.26(c)(3)(iii): The ladder base section must be placed with a secure footing. a. At the front of the building where an employee washed windows from the roof on a little giant portable ladder, the ladder was not set on a level foundation or secured from falling when the employee was stradling the top of the ladder thereby exposing the employee to a 21 foot fall hazard.

29 CFR 1910.26(c)(3)(vii): Ladders should not be used as a brace, skid, guy or gin pole, gangway, or for other uses than that for which they were intended, unless specifically recommended for use by the manufacturer. a. On the front roof of the Columbus Club where an employee was working from an extension ladder to wash windows, the ladder was not used in the maner it was designed for in that the ladder was not to be used on a roof without proper set-up thereby exposing employees to a fall hazard. b. On the front of the Columbus Club where an employee was working from a Little Giant portable ladder, the employee was straddling the top of the ladder to wash windows thereby exposing the employee to a 21 foot fall hazard.
