29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: a) On or about April 30, 2012, located in the Roll Draw Abrade (RDA) Department the employees used a roll polisher machine to rotate part at a rate of 1,087 RPM to polish the outer center bevel groove of the roll tool. The area between the part and the chuck was not protected from the entanglement hazard created by the rotating chuck, the part, and the hand held nylon rope media and exposed operator to hand entanglement from the rotating parts..
