29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: East end of building interior, southeast area of the worksite, toggle switch: On 05/10/2012, employees were exposed to shock hazards from contact with energized, exposed terminals while operating the toggle switch for the masonry table saw. The switch missing a cover was installed inside a junction box attached to the equipment; at a height of 42 inches above the ground, and operated at 120 volts AC.
