29 CFR 1910.146(k)(2)(iv): An employer whose employees have been designated to provide permit space rescue and emergency services did not ensure that affected employees practiced making permit space rescues at least once every 12 months: (a) The attendant, who was expected to perform rescue, had never been trained to conduct a rescue or practice a rescue; (b) The entrant, who was expected to perform rescue, had never been trained to conduct a rescue or practice a rescue.
