29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: Gonzalez Roofing at 1295 Hawthorne Ave., Boulder, CO 80304: On and before May 2, 2012 the employer did not ensure that employees were provided eye protection when pneumatic nailers were used during roofing operations. Condition exposed six employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: Gonzalez Roofing at 1295 Hawthorne Ave., Boulder, CO 80304: On and before May 2, 2012 the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities on a roof with a fall hazard of approximately 9-10 feet without the use of a fall protection system. Gonzales Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 109218, Citation Number 1, Item 1 issued on March 16, 2012 and became Final Order on April 17, 2012 with respect to a workplace located at 3746 Higgins St., Loveland, CO 80538.
