29 CFR 1910.141(d)(2)(ii):     Each lavatory shall be provided with hot and cold running water, or tepid running water.    At the work site, employees worked as chair-side and dental assistants.  In the public and employee restrooms, there was no tepid or warm running water for employees to wash their hands.

29 CFR 1910.1030(g)(2)(ii)(A):     The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.  Training shall be provided as follows: At the time of the initial assignment to tasks where occupational exposure may take place.      a.  At the work site, employees working as chairside assistants and dental hygenists were not provided training on bloodborne pathogens.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.      a.  At the work site, employees who worked as chair-side and dental assistants cleaned patient areas with disinfectants and virucides that included, but were not limited to, phosphoric acid-based gel etchant, Cavicide surface disinfectant/decontamination cleaner, and Maxicide Plus 28-day, 3.4% Long-Life glutaraldehyde.  Employees had not been trained on the hazards of the chemicals they used daily to perform their jobs.

29 CFR 1910.1200(e)(1)(i):     Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    A list of the hazardous chemicals known to be present using an identity that is referenced on the appropriate material safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas).    a.  At the work site, employees who worked as chair-side and dental assistants used hazardous chemicals such as but not limited to disinfectants and virucides to clean patient areas.  There was no listing of hazardous chemicals in the manual made available to employees.
