29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: The employer does not ensure all electrical power circuits employees work near are de-energized and grounded or effectively guarded by insulation or other means. This violation occurred on or about May 15, 2012, when employees working in an office building being remodeled were exposed to electrical hazards while installing and relocating lighting fixtures in close proximity to live, unguarded electrical circuits.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: The employer does not ensure all employees are trained to recognize and minimize the hazards of working on ladders. This violation occurred on or about May 15, 2012, and at times prior thereto, when employees working in an office building being remodeled were not trained to safely use the ladder while installing or relocating lighting fixtures.
