29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    Along the Bi-Lo store-front, on or about May 15, 2012, hard hats were not worn by employees who were exposed to being struck-by construction tools and materials while working below elevated work.

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    Along the Bi-Lo store-front, on or about May 15, 2012, employees who were working while standing in an aerial lift platform were not protected with a harness and lanyard that was tied-off to the platform, resulting in an eleven-foot fall hazard.    Abatement Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    Along the Bi-Lo store-front, on or about May 15, 2012, no guardrails or other forms of fall protection were provided for employees who were working while standing on the roof, resulting in a fourteen-foot fall hazard.
