29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a/ At the job site, one employee climbed over a wooden, standard guardrail system supported by the elevator equipment room platform then stepped on to a walking/working deck supported by outriggers attached to a tubular weld walkthrough frame scaffold. The walking/working deck was approximately 15 feet above the ground.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a/ At the job site, four, wooden 2 X 10 scaffold planks comprising the northern end of the walking/working deck of the tubular weld walkthrough frame scaffold were severely damaged. Specifically, large cracks ran along much of the length of each planks and pieces of wood were missing. The walking/working deck was approximately 12 feet above the ground.
