29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    a)  Kevin Strickler dba Strickler Enterprises, Inc., on or about May 15, 2012, at the address of 700 Cherry Street, Galion, OH: The employer did not ensure employees were protected from falls while installing roofing material from a residential structure.  Employees were exposed to approximately a 15 ft fall off the roof edge to the drive way below.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  Kevin Strickler dba Strickler Enterprises, Inc., on or about May 15, 2012, at the address of 700 Cherry Street, Galion, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative acceptable measures while performing residential roofing operations.
