Observed on or about 4/30/2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.37(a)(3): Exit routes were not free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp were not provided where the exit route was not substantially level. Three emergency exit routes using folding escape ladders with metal rungs and fabric rails over the interior side of an eight foot brick perimeter security fence were not free and/or unobstructed. The folding ladders were emergency exit routes for workers who may be isolated between the brick security fence and a catastrophic event, such as with the eighteen 30,000 gallon LPG tanks. There also was no means provided for descending the eight foot wall on the exterior side. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices. a) Jobsite: Eagle Terminal, 520 Milford Road, Uwchland, PA 19480 - Testing, setting and cleaning of pressure relief devices for the LPG storage tanks did not follow recognized and generally accepted good engineering practices including but not limited to the State of Pennsylvania Code for Unfired Pressure Vessels, REGO (pressure relief device manufacturer) Pressure Pressure Relief Valve Repair and Testing, UL- 132 Safety Relief Valves for Anhydrous Ammonia and LP-Gas, CGA S-1.3 Pressure Relief Device Standards, and Enterprise Products Inspection and Testing of Pressure Relief Devices Standard, STD 9501. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
