29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Located at New England Boat Works, Inc., 1 Lagoon Road, Portsmouth, RI: On or about 04/13/2012 the companys energy control procedures (Lockout/Tagout program) did not include energy control procedures for all hazardous energy sources such as, but not limited to, energized parts, stored pneumatic pressure, gravity, and/or radiant heat for the following equipment: (a) CNC Milling Machine, Building 10, 2nd Floor: Procedures were not developed and used to control electrical energy and the pneumatic energy sources; (b) Small Parts Oven, Building 10, 2nd Floor: Procedures were not developed and used to control electrical energy and radiant heat energy sources; (c) Powered Industrial Trucks, Yard: Procedures were not developed and used to control electrical energy, blocking of gravity related hazards, and prevention of movement of the fork trucks while they are being serviced.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: Located at New England Boat Works, Inc., 1 Lagoon Road, Portsmouth, RI, Yard: On or about 4/13/2012, employees were operating the Manitou forklift using 10 foot fork extensions whereby the employer did not obtain the manufacturers written approval.
