29 CFR 1926.405(d): Switchboards and panelboards. Switchboards that have any exposed live parts shall be located in permanently dry locations and accessible only to qualified persons. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. Exposed blades of knife switches shall be dead when open. a. On or about May 16, 2012 - at the above addressed jobsite, a live electrical panel was not protected with a cover, thereby exposing employees to arc flash/blast hazards. b. On or about May 16, 2012 - at the above addressed jobsite, unused openings in the live electrical panel box were not guarded to prevent employee contact, thereby exposing the employees to electric shocks.

29 CFR 1926.405(g)(2)(iii): Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. a. On or about May 16, 2012 - at the above addressed jobsite, a flexible cord with visible splices was used to power a temporary light, thereby exposing the employees to hazards associated with electrical shock.
