Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH 43612; On or about July 12, 2012, the employer did not assure procedures were developed and utilized for the process of changing out dies for including, but not limited to, the 150T Minster and the 400T Blow Mechanical Power Presses. Employees were exposed to amputation hazards while changing out dies.

Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH 43612; On or about July 12, 2012, the employer did not assure employees were protected from caught by and amputation hazards while using the 1050 Microcut Milling Machines with unguarded chucks to drill holes into metal parts.

29 CFR 1910.217(b)(7)(iii):     Mechanical power press(es) using part revolution clutches were not provided with a means of selecting off, inch, single stroke, and continuous (when the continuous function was furnished) with the clutch/brake control to select the type of operation of the press;    a.) Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH 43612; On or about July 12, 2012, the employer did not assure the means of selecting the Off, Inch, Single Stroke, and Continuous modes, respectively, was capable of being adjusted only by supervision of the employer. Employees were exposed to amputation hazards from leaving the key in the mode selector switch in presses including, but not limited to the 200T Blow, 300T Blow, and 200T Clearing Mechanical Power Presses.  	  b.) Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH 43612; On or about July 12, 2012, the employer did not assure the means of selecting the Off, Inch, Single Stroke, and Continuous modes, respectively, was capable of being adjusted only by supervision of the employer. Employees were exposed to amputation hazards when no key was required to change the operating mode on the 150T Minster Mechanical Power Press.

Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH 43612; On or about July 12, 2012, the employer did not assure employees were trained in the safe method of work with adequate supervision to assure that correct operating procedures were being followed. Employees were exposed to amputation hazards from leaving keys in the mode selector switch on presses including, but not limited to the 200T Blow, 300T, and 200T Clearing Mechanical Power Presses.    b.) Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH 43612; On or about July 12, 2012, the employer did not assure employees were trained in the safe method of work with adequate supervision to assure that correct operating procedures were being followed. Employees were exposed to amputation hazards from using a broken mode selector switch that no longer required a key on the 150T Minster Mechanical Power Press.

29 CFR 1910.217(e)(3):     Original and continuing competence of personnel maintaining mechanical power press(es) was not insured:    a.) Quality Tool Company located at 577 Mel Simon Dr. Toledo, OH; On or about July 17, 2012, the employer did not assure employees were properly trained to inspect and maintain mechanical power presses. The original and continuing competence of personnel performing power press inspections was not maintained in that employees operated presses that were not guarded properly. Employees tasked with maintaining power presses conducted inspections, but did not include all safeguards intended to protect employees from crushing or amputation hazards.
