On or about May 14, 2012, and at times prior thereto, in the production area of the Price Log Pro facility in Malvern, Arkansas, the employer did not pay for personal protective equipment required under the employer's hazard assessment. This exposed the employees to arc and splatter burn hazards.

The employer did not ensure that employees utilized proper procedures to shut down the Metal Muncher Hyrdaulic Fabrication Center prior to performing die changes. This practice exposed employees to the hazard of being caught in the machine in the event of an unexpected start up.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: On or about May 14, 2012, and at times prior thereto, in the production area of the Price Log Pro facility in Malvern, Arkansas. The employer did not ensure that employees shut off the energy isolating device on the Metal Muncher Hydraulic Fabrication Center prior to changing the dies. This practice exposed the employees to the hazard of being caught in the machine in the event of an unexpected start up.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects: On or about May 14, and at times prior thereto, terminals for welding leads were not protected from accidental electrical contact by personnel working in the production area of the Price Log Pro facility in Malvern, Arkansas. The employees were using Lincoln Ideal Arc CV400 welding machines to fabricate equipment used in the wood chip and logging industries. There were no covers over the live terminals of the welding machines. This practice exposed the employees to the hazards associated with electrical shock in the following: a) The welding machine marked #2. b) The welding machine marked #5. c) The welding machine marked #8.
