Building 7, Powder Coating - Company employee conducted powder coating operations in the Company's powder coating booth throughout each workshift. On or about, and dates prior to, August 10, 2012, electrostatic hand-held spray guns were operable when the ventilation fans were not in operation.

29 CFR 1910.134(i)(5)(iii): The compressor's sorbent bed and filters were not maintained and replaced or refurbished periodically as instructed by the manufacturer: a.) Building 7, Supplied Air System, East side of Powder Coating Booth The catalyst filter installed in a compressor line, used to supply breathing air to supplied air respirators, was not replaced as directed by the manufacturer. The manufacturers instructions stated that the catalyst filter must be changed at least every 12 months. On or about May 17, 2012, the documented date of the last catalyst change was October 2, 2010.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that compressors used to supply breathing air to respirators had tags containing the most recent change date and the signature of the person authorized by the employer to perform the change: a.) Building 7, Supplied Air System, East side of Powder Coating Booth On or about May 17, 2012, the Final charcoal filter on an air compressor line, which was used to supply air to supplied air respirators, was not tagged with the most recent change date. The date listed on the final filter was September 30, 2011. However, company work orders showed the final filter had been last changed on or about February 27, 2012. a.) Building 7, Supplied Air System, East side of Powder Coating Booth - On or about May 17, 2012, the Final charcoal filter on an air compressor line, which was used to supply air to supplied air respirators, was not tagged with the signature of the person authorized by the employer to change sorbent beds and/or filters. The employer required employees to wear supplied air respirators during powder coating operations.

29 CFR 1910.134(e)(6)(i): The employer did not obtain a written recommendation from a physician or other licensed health care professional regarding the employee's ability to use a respirator: a) Powder Coating- The physician's written opinions, maintained by the employer, did not contain the information required by29 CFR 1910.134. The documents stated only that respirator use was "Approved with no restrictions". The employer required employees to wear supplied air respirators during powder coating operations.
