29 CFR 1926.501(b)(11): Employees on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Zimmerman's Construction, LLC, 113 Manheim Street, Mount Joy, PA - Two (2) employees were removing roof tiles from an approximate 8:12 steep-sloped roof without an adequate and complete fall protection system, exposing the employees to an approximate 22-24 foot fall hazard, on or about May 16, 2012. No abatement certification or documentation required.

29 CFR 1926.602(c)(1)(vi): All industrial trucks in use shall meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation as defined in the American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks: (a) Zimmerman's Construction, LLC, 113 Manheim Street, Mount Joy, PA - An operator was not posted at the controls of a Sky Trak rough-terrain forklift (Model 6036), while the forks were used to support a raised platform for an employee. The employee was exposed to an approximate 11-13 foot fall hazard, on or about May 16, 2012. (b) Zimmerman's Construction, LLC, 113 Manheim Street, Mount Joy, PA - An employee was being lowered from the edge of an 11-12 foot high roof to the ground with the use of a forklift scaffold platform that extended over the 112-inch wheel base of a Sky Trak rough-terrain forklift (Model 6036) by approximately 4.5 feet on each side. This situation exposed the employee to an equipment stability hazard, on or about May 16, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
