29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:            a) Exterior Rear of Structure - Laborers and masons were observed stepping on cross bracing while gaining access                  to a scaffold work platform that was located 19 ft. 6 in. above the ground, .

29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:             a) Exterior Rear - Laborers and masons exposed to flying mortar, chips and dust were observed removing mortar                from existing brick joints and not wearing eye protection.     Renaissance Development was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 311770705, citation number 1, item number 1, and was affirmed as a final order on 03/25/08, with respect to a workplace located at 1919 16th Street, NW Washington DC.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:    a) Exterior Rear - Laborers and masons were observed on a tubular welded frame scaffold that measured 29 inches wide, 14 ft, in length, 28 ft, in height, with the work platform, 19 ft, 6 inches above the ground, while removing existing brick mortar joints, and the scaffold was not secured to prevent displacement, on or about 05/17/2012.
