Location: Production Floor, 1380 Sheridan St, Chicopee, MA    On 5/16/11, employees whose job required them to work in areas in which servicing or maintenance of machinery was being performed were not instructed in the purpose and use of the company's energy control procedures, exposing them to hazards.

29 CFR 1910.212(a)(2):     Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    Location: Production Area, 1380 Sheridan St, Chicopee, MA    On 5/29/12 and at times prior, two instances of lack of secured machine guarding exposed employees to in running nip point hazards.    a)  On 5/16/12, an employee was seriously injured when his arm was pulled into the infeed side of the Midwest Automation PR 500 Pinch Roller machine while the interlocked stop guard was placed in an inoperative position.     b)  Employees were exposed while working adjacent to the removed Outfeed Lower Roller Nip Guard on the Black Bros. Co. Adhesive Roller machine.
