(a) South Side of the Building -  An owner/foreman and a carpenter were not using fall protection while going between the JLG Lift and the roof where they were working exposing them to a 25 foot fall hazard, on or about 05/17/12.     (b) 1st Floor, Inside the Main Entrance to the Building - A foreman was not using fall protection while retrieving the water hose which was placed next to an unguarded edge ,exposing him to an 8' fall hazard on or about 05/17/12.     (c) (b) 1st Floor, Inside the Main Entrance to the Building - A superintendent was observed without fall protection while walking past and working near an unprotected edge exposing him to an 8' fall hazard, on or about 05/17/12.
