29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    On or about May 09, 2012, the employer failed to ensure that:      a) a household use power strip was used in the manner in which it was intended.   b) a single gang plastic box was used in the manner in which it was intended.     Employees were allow to utilize a power strip which was plugged into the single gang plastic box as a power source for a Mikita circular saw at the residence that was being renovated. The single gang box was connected directly to the power drop down with romex wiring.

29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    On or about May 9, 2012, the employer failed to ensure that the electrical panel was equipt with ground fault circuit interrupters (GFCI).  Employees were allow to utilize a power strip which was plugged into the single gang plastic box as a power source for a Mikita circular saw at the residence that was being renovated. The single gang box was connected directly to the power drop down with romex wiring.

All 120-volt, single phase, 15- and 20- ampere recepticale outlets on the consturction site, which wre not parat of the permanent wiring of the building or structure and which were in use by employees, did not have, in operating condition, approved Ground Fault Circuit Interrupters (GFCI) for personnel protection.     On or about May 09, 2012, the employer failed to ensure that Ground Fault Circuit Interrupters (GFCI) for personnel protection were provided. Employees were allowed to utilize electrical power from a single gang plastic box. The single gang plastic box did not have a GFCI.

29 CFR 1926.404(b)(1)(iii)(C): An assured equipment grounding conductor program was not being utilized, equipment found to be damaged or defective was being used prior to being repaired:    On or about May 09, 2012, the employer failed to ensure that equipment found to be damaged was repaired prior to use. Employees were allowed to utilize an extension cord that was damaged due to overcurrent at the residence that was being renovated.

Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed.    On or about May 09, 2012, the employer failed to ensure that the electric panel was equip with a dead front. Employees were utilizing the electric panel as a power source for electrically powered equipement.
