29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose: (a) At the worksite, on May 16, 2012, an indoor Slater Pass & Seymor brand electrical outlet box was laying on the ground between the north and south apartment complexes. The outlet box was not equipped with a bubble cover or hood cover. The outlet box was not equipped with a bubble cover or hood cover. This condition exposed employees to electrical shock. (b) At the worksite, on May 16, 2012, an indoor Slater Pass & Seymor brand electrical outlet box was nailed to the telephone pole between the the north and south apartment complexes. The outlet box was not equipped with a bubble cover or hood cover. The outlet box was not rated for outdoor use. This condition exposed employees to electrical shock and burns.

29 CFR 1926.405(a)(2)(ii)(A): Temporary wiring feeder conductors were not run as multiconductor cord or cable assemblies or within raceways; or, where not subject to physical damage, run as open conductors on insulators not more than 10 feet apart: (a) At the worksite, on May 16, 2012, a Southwire electrical conductor was routed from a southern circuit panel through a window to a receptacle box inside an apartment. The electrical conductor was not provided any protection from physical damage. (b) At the worksite, on May 16, 2012, a Southwire electrical conductor was routed from a northern circuit panel through a window to a receptacle box inside an apartment. The electrical conductor was not provided any protection from physical damage. (c) At the worksite, on May 16, 2012, a Southwire electrical conductor was routed from a northern circuit panel on the ground to a receptacle box mounted onto a telephone pole . The electrical conductor was not provided any protection from physical damage. (d) At the worksite, on May 16, 2012, a Southwire electrical conductor was routed from a middle-wall circuit panel through a window to a receptacle box inside an apartment. The electrical conductor was not provided any protection from physical damage.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: (a) At the worksite, on May 16, 2012, a powered temporary outlet box on the ground was left uncovered. This condition exposed employees to electrical shock. (b) At the worksite, on May 16, 2012, a powered temporary outlet box nailed to the eastside telephone pole was left uncovered. This condition exposed employees to electrical shock. Hurricane Electrical was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.405(b)(2), which was contained in OSHA inspection number 31196077citation number 1 item number 2 and was affirmed as a final order on May 6, 2009, with respect to a workplace located at 2806 Miller Ranch Road, Pearland, TX 77584.
