Condition exposed employees to "crushing" injuries associated with inadevertent contact between moving and stationary components.    (b)  On and before April 2, 2012, the employer did not ensure the lower blade guide pulley assembly under the DoAll vertical band saw was covered to prevent accidental contact with the rotating pulley. Condition exposed employees to inadvertent contact with the rotating pulley and/or in-running nip point between the blade and pulley. During a walkaround inspection on May 15th, 2012, an open ground condition was discovered using a Woodhead brand, model (1750), testing device.
