29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the forks.    a) Northwest side of the building: A wooden shipping pallet used for lifting personnel was only slipped on top of the forks of a Lull telescopic forklift, ID number FL5 and it was not secured to the forks. Employees were exposed to falls of up to 24 feet.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.     a) Northwest side of the building: Employees working from a scaffold plaform placed on the forks of a forkllift had not been trained to recongnize hazards associated with that scaffold, as evidenced by: The scaffold platform not being equipped with guardrails; The scaffold platform not being properly constructed; and the platform not secured in place.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a) Northwest side of the building on the second floor: Employees working 19 feet above ground level had not been trained by the employer on how to recognize what a fall hazard is, and how to minimize the hazards and protect themselves from falls.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) 2nd floor doorway opening, northwest side of the building: Employees working in the doorway opening were exposed to falls of 19 feet, and they were not protected from falling by using personal fall arrest stystems, guardrail systems or other means.
