29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Jobsite, 915 Ridge Avenue, Pittsburgh, PA: On or about May 3, 2012, during masonry activities at the west end of a scaffold, the employer did not ensure that the scaffolding system leading from the main building to the work area on the scaffold was provided with an appropriate guardrail system in that the top rail was too low, exposing employees to fall hazards of approximately 45 feet. b) Jobsite, 915 Ridge Avenue, Pittsburgh, PA: On or about May 3, 2012, during masonry activities at the north side of the scaffold, the employer did not ensure that the scaffold had a midrail along the working platform, exposing a worker to a fall hazard of approximately 45 feet. c) Jobsite, 915 Ridge Avenue, Pittsburgh, PA: On or about May 3, 2012, during masonry activities at the northeast end of the scaffold, the employer did not ensure that the scaffold outrigger platform was provided with an appropriate guardrail system. An outrigger bracket was used for fall protection here that did not have an adequate height for a toprail and there was no midrail, exposing employees to a fall hazard of approximately 45 feet.
